Job Summary
The Community Care Associate plays a vital role in enhancing the quality of life for individuals with intellectual disabilities by providing home-based therapy and intervention. This position requires assisting clients in daily living activities, following care plans developed by social service and/or healthcare professionals.
Main Responsibilities
- Maintain Client Well-being: Provide Home-Based Therapy and Intervention, assisting clients in daily living activities, following care plans developed by social service and/or healthcare professionals.
- Clinical Documentation: Maintain up-to-date documentation of clients' conditions and provide inputs for the development and review of care plans.
- Independence Promotion: Conduct activities to promote independence, health, wellness, and quality of life; guide clients on personal and social skills, self-care, and community living skills.
- Training and Support: Provide client training and support, conduct programs and activities, communicate effectively with caregivers, and work on training plans.
Requirements
- Certification and Experience: Minimum ITE certification. Experience in education/training of persons with intellectual disability or in a multi-disciplinary team environment is an added advantage.
- Personal Qualities: Self-motivated and able to participate in on-site and off-site activities.
