Overall Job Responsibility
You will provide case management services to enable persons with disabilities to better prepare for and transition into employment. Your role includes conducting outreach, working closely with stakeholders such as employers to promote inclusive hiring, and managing transition programs. You will also develop SOPs, collaterals, and publicity materials.
Duties and Responsibilities
Manage cases and develop support strategies to help clients secure and retain employment
Assist clients in leveraging their support networks, including co-workers, caregivers, and the community
Impart social and soft skills to facilitate client integration and performance at work
Engage employers to promote inclusive hiring and market our programs and services
Collaborate with government agencies, hospitals, VWOs, and relevant organizations to facilitate employment transitions
Conduct outreach to hospitals and agencies to raise awareness of programs
Address stakeholder concerns and foster mutually beneficial relationships
Create collaterals and publicity materials for programs
Perform additional roles as required
Qualifications, Knowledge, and Experience
Diploma or Degree in Social Work, Psychology, Healthcare, or related fields
Experience in social services, healthcare, or public services preferred; fresh graduates are welcome
Ability to meet tight deadlines and follow up diligently
Team player with adaptability and versatility
Strong problem-solving, communication, and interpersonal skills
Proficiency in languages beyond English (e.g., Mandarin, Malay) is an advantage
Proficient in Outlook, MS Word, Excel, PowerPoint
