Community Change Maker

We are seeking a dedicated and compassionate Community Change Maker to play a vital role in driving our mission to bring positive change into the lives of vulnerable families and individuals.

As a key member of our team, you will be responsible for conducting intake assessments, providing emotional support and guidance, and facilitating access to external resources and services.

Key Responsibilities:

  • Develop strong relationships with clients and other key stakeholders through effective communication and interpersonal skills.
  • Conduct thorough needs assessments and risk evaluations for complex cases, working closely with relevant agencies as needed.
  • Prepare comprehensive social reports and case recordings, ensuring timely updates to our database.
  • Collect and organize relevant documents from clients, meeting deadlines for report submissions and audit preparation.
  • Work collaboratively with clients to set and review goals, implementing intervention plans tailored to their unique needs.

Accountability:

  • Report directly to the Team Lead and Director of Social Services.

    • Ability to work with vulnerable populations.
    • Case management and casework knowledge and skills.
    • Risk assessment and risk management.
    • Good report writing skills.
    • Self-motivated individual with the ability to work independently.
  • Technical Competencies:
    • Proficiency in IT Systems and Microsoft Office Suite.
  • Education and Experience:
    • Degree in Social Work from an accredited institution.
    • Accredited as Registered Social Worker with SASW.
    • At least 2 years of relevant working experience in similar sectors.
