Job Title: Therapy Services Specialist
Role SummaryThe successful candidate will be responsible for reviewing and assessing patients to develop effective treatment plans, focusing on delivering exceptional care and promoting overall well-being.
- Assess patient needs and develop personalized treatment plans
- Implement individualized care plans and provide basic administration duties related to patient-care
- Collaborate closely with the Home Care Team to coordinate patient-care, including scheduling appointments and monitoring frequency
- Develop and implement high-quality treatment plans that meet the unique needs of each patient
- Provide exceptional patient care and promote overall well-being through evidence-based practice
- Cultivate strong relationships with patients, families, and healthcare teams to ensure seamless coordination and communication
- Bachelor or Master's degree in Physiotherapy is required for this position
- Must be fully registered with the Allied Health Professions Council (AHPC) in Singapore to ensure compliance with regulatory standards
- Project commencement date is September 2025, with negotiable start dates to accommodate suitable candidates
- Strong interpersonal skills, enabling effective communication and collaboration with diverse stakeholders
- Proven expertise in quality improvement and treatment planning, with a focus on delivering high-quality patient outcomes
- Experience in musculoskeletal, occupational therapy, and rehabilitation settings, with a demonstrated ability to adapt to changing situations
