Overview
Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies, procedures and training and in compliance with regulations set forth by the corporation, state and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI activities, including those related to patient satisfaction, and actively participates in process improvement activities that enhance the likelihood that patients will achieve FMC Quality Enhancement Goals (QEP).
Duties / Activities
Customer Service :
Responsible for driving the FMCNA culture through values and customer service standards.
Accountable for outstanding customer service to all external and internal customers.
Develops and maintains effective relationships through effective and timely communication.
Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.
Principal Responsibilities And Duties
PATIENT CARE :
Assist other health care members in providing patient-specific education regarding adequacy measures where appropriate (Online Clearance Monitoring, Adequacy Monitoring Program, Urea Kinetic Modeling) and disease process/access.
Welcome assigned patients and inquire as to their well-being since their last treatment; report any complaints or observations to the nurse supervisor.
Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
Obtain pre and post-treatment vital signs and weight; report unusual findings.
Perform vascular access evaluation pre-treatment and report unusual findings.
Perform vascular access cannulation and report any difficulties or unusual findings to nurse supervisor.
Administer Heparin as delegated or allowed by state law.
Initiate dialysis treatment according to orders (QB/QD).
Evaluate intradialytic problems and provide intervention as prescribed or directed.
Monitor patients' response to dialysis; report any unusual findings to nurse supervisor.
Obtain vital signs prior to reinfusion; document and complete related procedures for discontinuation of treatment.
Discontinue dialysis treatment according to established procedures.
Evaluate patient prior to termination of venous access (standing and sitting blood pressure).
Obtain hemostasis and apply appropriate dressings.
Evaluate patient prior to discharge and report any unusual findings to nurse supervisor.
Perform and record pre/post dialysis evaluation weight and vital signs with initial identification.
Calculate and enter dialysis machine programming for UF goal, treatment time, Na modeling, and UF modeling as prescribed.
Ensure safety alarms are enabled, including NVL 160.
Record accurate and timely information on the Hemodialysis Treatment Sheet with initial identification.
Document patient treatment information; complete Hemodialysis Treatment Sheet and progress notes as applicable.
Report significant changes in patient condition to the nurse supervisor.
Observe patient and perform machine safety checks; report changes or unusual findings.
Intervene for unusual patient status and document response.
Evaluate vascular access during treatment and provide interventions as needed; report findings to nurse supervisor.
Obtain prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE / TECHNICAL :
Maintain environmental integrity and aesthetics; keep areas safe and clean.
Clean and disinfect dialysis machine surfaces, chairs, equipment and surrounding area between treatments per policy.
Prepare for dialysis treatment and document where appropriate.
Perform pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
Initiate Solution Delivery System (SDS) system.
Ensure blood spills are cleaned with appropriate disinfectant per policy.
Prepare, organize and use supplies to prevent wastage.
CLERICAL & ADMINISTRATIVE :
Participate in Continuous Quality Improvement (CQI) activities and document progress.
Enter all treatment data into the designated clinical application accurately and timely.
Review treatment sheets for completeness, ensure nursing signatures, and correct omissions.
Prepare initial patient chart for admission and paperwork for chart placement.
Prepare lab requisitions for prescribed or stat lab specimens per destination.
Label, prepare and store lab samples per requirements; ensure lab specimens are collected by appropriate courier.
OTHER :
Perform additional duties as assigned.
Addendum for Home Programs
Assisting the HT RN: may assist on home visits and reinforce education under supervision.
Schedule and contact patients regarding appointments; weigh patient and obtain vital signs.
Collect treatment records, assist with lab work and home machine setup as directed.
Clerical duties and inventory management for home programs; assist with supply management and customer service.
Physical Demands & Working Conditions
The role involves lifting, standing, travel between facilities, and potential exposure to infectious materials. Reasonable accommodations may be provided. The job requires frequent standing, bending, and potential lifting up to 40 lbs with assistance.
Education
CMS-approved state and/or national certification or high school diploma or GED and must meet certification requirements within state or CMS timeline.
All appropriate state licensure education and training as required.
Experience And Required Skills
Previous patient care experience in a hospital setting or related facility preferred.
Continued employment is dependent on successful completion of FMCNA dialysis training program and CPR certification.
