Job Description: Clinical Pharmacy Technician

Job Overview:

The Virtual Care Centre delivers patient-centred, technology-enabled care to patients in the community. It is a medical helpline that supports patients and community partners.

Main Responsibilities:

  • Support tele-pharmacy sessions and coordinate medication fulfilment.

  • Carry out counselling for newly initiated medication by the Virtual Care Centre.

  • Address basic patient queries regarding medication handling, dosing schedules, supply status and escalating complex cases to the pharmacist.

  • Assist pharmacists in history taking and structured medication reconciliation for requests related to medication top-ups.

  • Process prescriptions for Virtual Care Centre patients and coordinate with procurement to support supply of non-formulary medications.

  • Facilitate timely home delivery of medications and work with pharmacy home delivery team and external vendors to track, confirm and troubleshoot deliveries.

Key Qualifications and Skills:

  • Possess Certified Pharmacy Technician Certification (CPTC) or Diploma in Pharmaceutical Science.

  • Familiarity with pharmacy and hospital IT systems.

  • Strong coordination, communication and interpersonal skills to work in a multidisciplinary care team.

  • Highly organised and able to manage multiple responsibilities in a fast-paced, evolving care setting.
