**Job Summary**:
To support Rehab Counsellors in:
b. Executing day-to-day case management tasks

**Duties and Responsibilities:
Medifund

1. Collecting Supporting Documents from residents.
- In January and July for residents brought forward from previous Medifund period.
- Immediately upon admissions.
- On a monthly basis for changes.

2. Checking and updating Centre’s fees payment template with payments received on a daily basis.

3. Assist to collect fees from residents.

4. Fill-up and update relevant portion of Medifund Application form on a monthly basis.

5. Review the accuracy of payments received the previous month by comparing Centre’s fees payment template with Corporate Finances’ Aging Report by the 15th day of the following month; troubleshoot and identify errors when detected.

7. Preparing and submitting Medifund statistics. projections, and reports to AIC/MOH and Corporate Finance on a timely manner.

Case Management

1. Provide care coordination

2. Troubleshooting and resolving conflicts among residents

3. Network and work with referral source, caregivers, community agencies and partners, relevant government agencies, and other stakeholders to provide holistic care and support

4. Transition and discharge preparation

5. Maintaining case files to ensure that all required assessments and documentations are completed within the prescribed time-frame

6. Capture, collate and analyse relevant data on a timely manner for service review, statistical reports, etc.

7. Assist in other duties and functions as assigned by supervisor
