**THK**MC was incorporated on 13 October 2011 as a charity to provide multiple social and welfare services to the community at large, and achieved an Institute of Public Character (IPC) status on 18 November 2011. **THK**MC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed and culture. **THK**MC today serves more than 70,000 beneficiaries through more than 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors' Community, and Therapy.

DESIGNATION : Assistant Case Manager

RESPONSIBILITIES
- Adopt a case management approach when working with clients ie. network with other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients
- Ensure proper documentation of client's progress and care plans
- Ensure all clients' records are appropriately kept
- In consultation with the Case Manager, provide training and supervision to CAN Carers including recruitment and orientation
- Communicate, supervise and support CAN Carers
- Monitor home visit schedule of CAN Carers
- Participate in outreach activities and events
- Collect statistics and data relating to service operations
- Assist with administrative duties relating to the Centre's operations
- Any other duties as assigned by the Center Director/ Social Worker / Case Manager

QUALIFICATIONS
- At least a Diploma in Social Service, Psychology, Counseling or any other related disciplines

OTHER INFORMATION
- Previous experience in social work or related field or work with older adults in long term care is desirable
- Proficient in speaking & writing good English. Proficient in speaking and writing a 2nd language and ability to speak dialects will be an added advantage.
- Must have a person-centered approach when assisting seniors
- Good written communication skills to document information in a clear, concise and accurate manner
- Possess excellent interpersonal skills
- Proficient in Microsoft office (Word, PowerPoint, Excel)
- Passionate individual with commitment in working with seniors and other marginalized groups.
- Resilient and positive team player
- Able to work independently and work in a multi-disciplinary team
- Compassionate Case Worker who is driven by deep values and beliefs that client is capable of change
- An enabler who take a series of action and strategies to enable clients to achieve solutions
- An individual who is able to manage emotions and stay composed and positive under difficult circumstances and responding of conflict. Able to think clearly and stay focused under pressure, adapt to harsh environment and thrives on continuous personal learning.
