Job Title:

Case Management Officer

Roles and Responsibilities:

  • Project Manager: Oversee the implementation of a comprehensive programme to demonstrate the effectiveness of early introduction of palliative care for advanced CKD patients, titled Project Renalssance.
  • Programme Development: Drive the development and implementation of Kidney Supportive Care clinics for the multidisciplinary management of advanced CKD patients.
  • Policies and Procedures: Develop and disseminate hospital policies, create standard operating procedures, protocols, and case management tools to ensure seamless patient care.
  • Mentorship: Provide guidance and mentorship to Case Management officers on project management and staff performance management.
  • Staff Performance: Monitor and evaluate staff performance, providing constructive recommendations for improvement.
  • Education and Training: Foster educational opportunities for staff development and contribute to the growth of the Division.
  • Coordination: Collaborate with the CMO to coordinate appointments to the Kidney Supportive Care Clinic and referrals to community partners.
  • Medico-Legal Issues: Apply knowledge of prevention of medico-legal issues in patient care.
  • Quality Improvement: Participate in quality improvement activities to enhance patient outcomes.

Requirements:

  • Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology, or General Science.
  • Certificate/Diploma in Renal Nursing is advantageous.
  • Registered Nurse with at least 3 years of clinical experience.
  • Minimum 5 years working experience in healthcare, with managerial duties.
  • Knowledge of clinical skills and critical thinking in patient care.
  • Ability to resolve operational, IT, and clinical issues.
  • Leadership skills to lead a team of Coordinators.
  • Interpersonal and communication skills.
  • Computer literacy – MS Office (Word, Excel, PowerPoint).
  • Effectively bilingual.
