Senior Clinical Program Manager

The ideal candidate will be responsible for managing the operational aspects of a program aimed at demonstrating the effectiveness of early introduction of palliative care options for patients with advanced Chronic Kidney Disease (CKD). The program, called Project Renalssance, aims to develop and drive the implementation of a Kidney Supportive Care clinic for multidisciplinary management of advanced CKD patients.
This role requires a high degree of expertise in clinical skills and critical thinking in patient care. The candidate must have experience in staff performance management and mentoring, as well as excellent communication and interpersonal skills. Additionally, they must be able to lead a team of coordinators in implementing the program, delivering key performance indicators, motivating staff, and ensuring knowledge retention and succession.
Key Responsibilities:- Develop and Implement Operational Plans: Develop and implement operational plans for Project Renalssance, including strategies for staff training, quality improvement, and patient outcomes.
- Mentor and Train Staff: Mentor and train Case Management officers on project management and staff performance management.
- Lead Team Efforts: Lead a team of Coordinators in implementing the program, delivering key performance indicators, motivating staff, and ensuring knowledge retention and succession.
- Monitor and Evaluate Performance: Monitor and evaluate staff performance, with follow-up on recommendations for improvement.
- Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology, or General Science.
- Certificate or diploma in Renal Nursing advantageous.
- Registered Nurse with at least 3 years of clinical experience.
- At least 5 years prior working experience in a healthcare environment, assuming managerial duties before.
- Knowledge of clinical skills and critical thinking in patient care advantageous.
- Ability to resolve problems in all aspects of program implementation.
