We are seeking a highly skilled and compassionate professional to fill the position of Case Manager. This role plays a vital part in improving the quality of life for patients with advanced Chronic Kidney Disease (CKD).

Key Responsibilities:
  • Coordinate patient appointments at the Kidney Supportive Care Clinic.
  • Deliver high-quality patient care according to established standards, utilizing patient care plans and clinical pathways.
  • Collaborate with physicians and other healthcare providers to ensure seamless patient care and services.
Requirements:
  • Degree in Nursing, Biomedical Science, Life Sciences, or General Science.
  • Certificate/Diploma in Renal Nursing is an advantage.
  • At least 3 years of clinical experience as a Registered Nurse.
  • Excellent interpersonal and communication skills.
  • Proficiency in MS Office applications.
  • Bilingualism is a plus.
