We are seeking a skilled and compassionate Case Manager to join our team in delivering mental health services. As a Mental Health Specialist, you will provide case management and counseling services to individuals with mental health conditions, dementia, and support for their caregivers.

  • Key Responsibilities:
  • Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualised care plans and interventions.
  • Provide case management, counseling, and care coordination for clients with mental health conditions.
  • Prior experience or knowledge in elderly mental health will be an advantage.
  • Plan, organise and implement community outreach, programs and group work.
  • Collaborate with community partners and stakeholders to support clients with complex health and social needs.
  • Deliver talks to raise awareness of mental health and dementia.
  • Support and manage volunteers who are befrienders to clients with mental health needs.
  • To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.

Required Skills and Qualifications:

  • Experience and/or Knowledge: Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
  • Clinical Skills: Strong interpersonal and communication skills.
  • Interpersonal Skills: Able to work independently and collaboratively within a team.
  • Educational Background: Good administrative and computer skills with proficiency in MS Office and Grad Diploma/ Degree in Social Work.
