Senior Case Coordinator

The role of a Senior Case Coordinator is to provide administrative support and ensure the smooth operation of our case management processes.
- Key Responsibilities:
- Collect and verify personal and financial information from residents and their families prior to the application of financial assistance schemes.
- Maintain accurate and up-to-date records in our referral and enquiries database and information system.
- Ensure timely submission of applications for financial assistance, including Medifund, MFEC, IDAPE, PGDAS, and others.
- Coordinate referrals and process applications for potential residents as the main point of contact and liaison between residents, families, and healthcare institutions.
- Document case files, referrals, and other relevant documents for effective case management and audit purposes.
- Perform data entry tasks and regularly update information to maintain the accuracy of our databases.
- Handle administrative tasks, including enquiries, referrals, and admissions.
- Facilitate residents' transfers to other facilities or healthcare institutions, and coordinate discharge processes.
- Requirements:
- High school diploma or equivalent required; degree preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Proficiency in Microsoft Office Suite, particularly Excel and Word.
