About the Role:

  • The primary function of this role is to facilitate family sessions and community engagement, employing evidence-based and culturally sensitive practices. This involves collating and consolidating information from relevant professionals and sources in the school.">
  • Establishing rapport with parents and utilizing various strategies to engage them is also a key aspect of this role.">
  • Facilitating parents' understanding of programme, profile of child in different settings, and triangulating information with recommendations from the team are additional responsibilities.">
  • This position requires conducting holistic bio-psycho-social assessments of children and families to identify needs, risks, and strengths for intervention.">
  • A significant part of this role is facilitating documentation needed for enrolment in programme and providing relevant resources for family/caregivers.">
  • Effective communication with parents in verbal and/or written form is essential.">
">Key Responsibilities:">
  • ">
  • Participate in case discussion by providing information about child and family strengths and needs and identifying possible care plans required for the child and/or family.">
  • Engage and work collaboratively with various community agencies (e.g., Family Service Centres, Social Services Offices, or other agencies) to ensure holistic case management for families.">
  • Establish resources available within caregivers' support system, in school and community required for child and family to maintain child's participation in school and DS-LS intervention to maximise child's potential.">

  • Completes documentation (e.g. case notes) accurately and communicates information effectively to relevant professionals.">
  • Evaluates and identifies areas required for follow-up (e.g. referral to appropriate internal and/or external resources).">

  • Demonstrates knowledge of available community resources.">
  • Utilises appropriate community resources to support child and/or family.">
  • Networks with all relevant stakeholders and partners (e.g., at local and/or regional level) for collaborative problem-solving.">

  • A diploma in social work service or early childhood education.">
  • A minimum of 2 years of experience within the community setting with an understanding of working with families with young children.">
