Job Title: Renal Care Specialist

We are seeking a highly skilled and compassionate Renal Care Specialist to join our team.

Job Description:

The successful candidate will be responsible for coordinating appointments, delivering patient care, and collaborating with multidisciplinary teams to ensure the best possible outcomes for patients with chronic kidney disease (CKD).

  • Coordinate appointments for patients with CKD at our Kidney Supportive Care Clinic
  • Deliver patient education, counseling, and support to patients and their families
  • Collaborate with healthcare providers to develop and implement individualized patient care plans

Key Qualifications:

  • Degree in Nursing, Biomedical Science, Life Sciences, or General Science
  • Registered Nurse with at least 3 years of clinical experience
  • Proven ability to work effectively in a team environment and communicate complex information to patients and colleagues
