Job Title: Renal Care Coordinator

As a key member of our multidisciplinary team, the Renal Care Coordinator plays a vital role in ensuring that renal patients receive high-quality care and support throughout their treatment journey.

Responsibilities:

  • Manage caseloads for patients with chronic kidney disease (CKD) to delay disease progression through education, follow-up appointments, and discussion with primary physicians.
  • Liaise with vendors, dialysis centers, patients, and caregivers regarding end-stage kidney disease.
  • Conduct Peritoneal Dialysis (PD) competency assessments for patients choosing PD.
  • Engage in Advance Care Plan (ACP) discussions for pre- and post-initiation dialysis patients in both inpatient and outpatient settings.
  • Monitor and track patient care, including hepatitis B vaccination, anemia management, calcium phosphate control, and nutrition status.
  • Track statistics on workload and quality outcomes, as well as monitor patients with dialysis-related issues.
  • Coordinate care for patients under the renal transplant waitlist, requiring yearly reviews at transplant centers.
  • Monitor inpatient dialysis-related complications and hospitalizations.
  • Collaborate with nephrologists, surgeons, dialysis nurses, medical social workers, dietitians, and pharmacists to ensure quality patient care.
  • Organize seminars and workshops on kidney disease.
  • Complete monthly reports, track workload, key performance indicators, and balance scorecards.
  • Filing and handling patient documents systematically, confidentially, and orderly to prepare for audits.
  • Submit monthly HD and PD patient statistics to the National Registry Disease Office (NRDO).

Required Skills and Qualifications:

  • Bachelor's Degree in Nursing.
  • 2 years of working experience, preferably in renal-related fields.
  • Basic IT knowledge required; good PC skills are advantageous.
  • Positive attitude and consistent good performance.
  • Good interpersonal and communication skills.
  • Ability to work independently.
  • Proactive and able to implement change.
  • Singapore Nursing Board (SNB) license.
