Holistic Palliative Care Professional

A compassionate and skilled nursing professional will be responsible for coordinating and implementing care plans for patients with life-limiting illnesses. This role requires a strong foundation in nursing theory, as well as expertise in providing holistic patient care.
Key Responsibilities:
- Collaborate with interdisciplinary teams to provide seamless medical and nursing care, pain management, and compassionate support for patients and families.
- Develop and implement individualized care plans based on patient needs and preferences.
- Provide education and support to patients, families, and caregivers on end-of-life care and hospice awareness.
- Conduct home visits to assess patient needs, provide clinical care, and make recommendations for further treatment.
Requirements:
- State Registered Nurse with valid Practising Certificate issued by Singapore Nursing Board (SNB) and at least 3 years of working experience.
- Experience in Oncology or Palliative Care is advantageous.
- Nurses with paediatric experience are also welcome to apply.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and make clinical judgements in a home-care setting.
Preferred Qualifications:
- Computer literate.
- Ability to communicate effectively with patients from diverse backgrounds.
