Palliative Care Navigator

We are seeking a highly skilled Palliative Care Coordinator to join our team. As a Palliative Care Coordinator, you will play a vital role in providing high-quality care and support to patients and their families.
">Key responsibilities include:
">- Care Navigation: Provide individualized assistance to patients and their families to overcome healthcare system barriers, improve continuity of care and quality of Palliative care.
- Care coordination: Ensure compliance to palliative care pathways and streamline care for inpatients and outpatients.
- Liaise with internal stakeholders and external partners to ensure smooth transitions of care.
- Perform outcomes evaluation of care pathways.
Requirements:
">To be successful as a Palliative Care Coordinator, you should have:
">- A bachelor degree in Health Sciences, preferably in Degree in Nursing or Allied Health relevant to the job.
- Relevant working experience in a similar setting preferred.
- Good interpersonal and communication skills with the ability to work independently and as part of a team.
The ideal candidate will possess strong organizational and time management skills, be able to work effectively in a fast-paced environment, and have excellent communication and interpersonal skills.
