Palliative Care Specialist Role
- Palliative care provider and member of interdisciplinary specialist palliative care team
- Collaborative working environment with healthcare professionals
Responsibilities:
- Socio-economic assessments: Evaluate patients' practical concerns, such as financial matters, at the end of life
- Counselling and support: Navigate personal and ethical dilemmas, cultural conflicts, and spiritual issues through face-to-face sessions or home visitations
- Broad community engagement: Collaborate with educational, religious, and social service organizations to achieve optimal end-of-life goals for patients and their families
- End-of-life preparation: Engage families' involvement, encourage visits, and prepare patients/families for the dying process
- Bereavement support: Provide information, education, and support to help families cope with grief and loss
- Interdisciplinary collaboration: Attend meetings with medical, nursing, and allied health colleagues to discuss patient management plans
- Financial and discharge planning: Counsel patients and family members on financial and care plans
- Field work: Visit patients in hospital wards and other social agencies to alleviate medico-social problems
- Reporting and case presentation: Prepare reports for management and present cases for discussion in departmental or inter-departmental meetings
- Professional development: Maintain networking with MSW, Counsellor, staff nurses, and relevant medical staff/authorities
- Ongoing learning: Participate in in-service education programs and ensure optimum transfer of learning
Requirements:
- Degree in Social Work or Master in Counselling
- Registration with Singapore Association of Social Workers or Singapore Association for Counselling
- At least 4-9 years of experience in a healthcare environment
- Bilingualism and ability to communicate in local dialects preferred
