Main Responsibilities:
  • Carry out home visits to chronic ill/palliative patients who are being referred.
  • Conduct thorough nursing assessments on patients to identify their needs and develop personalized care plans.
  • Coordinate with doctors to ensure medical assessments and follow-up visits are conducted as required.
  • Monitor patient medication compliance and adjust plans as necessary.
  • Perform various nursing procedures, including wound dressings and insertion of catheters.
  • Develop and implement nursing care plans to manage patient conditions.
Required Qualifications and Skills:
  • Hold a valid SNB (Singapore Nursing Board) certification and BCLS (Basic Cardiac Life Support).
  • Prior experience in palliative care is highly preferred.
