Job Opportunity: Supporting Individuals and Families
We are seeking a dedicated and compassionate professional to play a vital role in supporting individuals and families in need.
Key Responsibilities
- Intake: Manage referrals and applications for admission into our program, ensuring seamless integration of services.
- Casework & Counselling: Conduct thorough assessments, develop intervention plans, and engage clients in a professional and therapeutic manner to understand their needs and build rapport.
- Programme Development: Assist in planning and implementing programs and activities, including group work and outreach initiatives that foster community engagement and social cohesion.
- Post-Discharged Monitoring: Assess, develop, and implement discharge plans, and follow up with clients post-discharge to ensure continuity of care.
In addition to these key responsibilities, you will be responsible for maintaining accurate records, preparing reports, and participating in research studies to inform community wellbeing strategies.
