Job Title: Dedicated Homecare Professionals
Our organization seeks a highly skilled and compassionate Homecare Staff Nurse to join our team. This role is ideal for individuals who are passionate about delivering exceptional patient care in a home-based setting.
The successful candidate will be responsible for assessing new clients, developing personalized care plans, and conducting health screenings as needed. They will also provide education and support to clients and their caregivers, while working collaboratively with doctors, allied health professionals, and external partners to ensure seamless care coordination.
Key responsibilities include:
- Assessing client needs and developing tailored care plans
- Conducting regular health screenings and making referrals as necessary
- Providing comprehensive education and support to clients and caregivers
- Coordinating care with healthcare providers and external partners
- Building strong relationships with clients and their families
- Supporting self-management of health conditions
- Providing emergency or crisis care when required
To be successful in this role, you will need to possess:
- A minimum diploma in nursing and registration with the relevant nursing board
- Excellent communication and interpersonal skills
- The ability to work independently and as part of a team
- Strong organizational and time management skills
