Description
We seek an experienced Nurse Homecare Professional to join our team. As a key member of our Home Healthcare Team, you will play a vital role in providing high-quality care and support to our clients in the comfort of their own homes.
Your responsibilities will include:
- Conducting comprehensive assessments of potential clients and their home environments to identify their needs and develop personalized care plans.
- Collaborating with caregivers and family members to ensure seamless transitions throughout the care journey.
- Monitoring clients' conditions and adjusting care plans as needed to ensure optimal outcomes.
- Providing financial counseling and assistance with government subsidies to help clients access necessary resources.
- Managing client payments and issuing receipts for services delivered.
- Assisting doctors during home visits and developing discharge plans for clients who no longer require our services.
- Conducting health education sessions and training for caregivers in the community.
- Referring caregivers to relevant services and supporting them in navigating the eldercare landscape.
Requirements
To be successful in this role, you must possess:
- A Diploma or Bachelor's Degree in Nursing or equivalent qualification.
- Registration with the Singapore Nursing Board.
- At least 3 years of experience in a related field, with a strong background in home care services.
