Job Title:

Homecare Nurse Specialist

We seek a skilled Homecare Nurse Specialist to deliver high-quality care and support to clients in their homes.

  • Conduct comprehensive assessments of potential clients and develop personalized care plans.
  • Collaborate with caregivers and family members to ensure seamless transitions throughout the care journey.
  • Monitor clients' conditions and adjust care plans as needed to achieve optimal outcomes.
  • Provide financial counseling and assistance with government subsidies to help clients access necessary resources.
  • Manage client payments and issue receipts for services delivered.
  • Assist doctors during home visits and develop discharge plans for clients who no longer require our services.
  • Conduct health education sessions and training for caregivers in the community.
  • Refer caregivers to relevant services and support them in navigating the eldercare landscape.

To be successful in this role, you must possess:

  • A Diploma or Bachelor's Degree in Nursing or equivalent qualification.
  • Registration with the Singapore Nursing Board.
  • At least 3 years of experience in a related field, with a strong background in home care services.
