Key Responsibilities
  • Strategic Collaboration: Identify opportunities for strategic collaborations with key partners to improve care integration and augment community care.
  • Population Health Strategies: Support the implementation of Population Health strategies and regional health plans.
  • Project Management: Regularly engage and project manage with key account agency/community/internal partners on collaborations.
  • Stakeholder Engagement: Keep abreast of developments within the social and health landscape.
  • Team Support: Support the team in development, implementation, data collation, evaluation, and reporting of initiatives and collaborations with agency partners, social care players, and cross-teams.
  • Event Management: Organize meetings and events.
  • Administrative Support: Take minutes for meetings and events and follow up on issues raised.
Requirements
  • Education: Degree required.
  • Experience: 1-2 years of working experience, prior experience in community care or strategic planning will be an advantage.
  • Skills: Experienced in project management and working with multiple stakeholders, good communication, interpersonal, and organizational skills, ability to multitask and work independently.
