Lead Community Care Team Operations

The ideal candidate will lead community care team operations in an assigned area to improve health and anchor care in the community. This role requires a strategic thinker with excellent leadership skills.

  • Key Responsibilities:
    • Support community care team operations in an assigned area to support residents in leading healthy lifestyles, including transitioning care programs, community health posts, and other initiatives.
    • Engage community partners in assigned areas to collaborate on taking care of residents' health and coordinate community operations.
    • Support operational planning by analyzing data, monitoring workload, and tracking performance metrics.
    • Improve community operations to enhance care and residents' experiences by piloting new programs and processes, evaluating technology use, and assessing pilot program success.
    • Enhance overall patient experience efforts to improve residents' experiences and quality of care by managing feedback, investigating service recovery, and evaluating feedback to identify potential service gaps.

Required Skills and Qualifications:

The successful candidate must have:

  • A Bachelor's degree or higher in a relevant field.
  • Relevant experience in healthcare, operations, or customer service environments.
  • At least 2 years of experience in a similar role.
  • Strong communication, interpersonal, and analytical skills.
  • Ability to manage complaints and feedback effectively.
  • Proficiency in Microsoft Office, especially Excel, and strong data analysis skills.
