The Health Management Executive is tasked with monitoring the general health of seniors under the care of REACH. He/she conducts community screening and outreach, enrollment of Seniors into the health management plans, and performs vital sign monitoring. He/she will accompany CREST teams and volunteer befrienders for joint assessments and support outreach.

**Responsibilities**:

- Provide health awareness/basic health monitoring and management services, and information to clients who are managing chronic illnesses, including those identified to be at-risk and their caregivers.
- Keep track and support Seniors health Plans and flag out non-compliance or other concerns to social workers / senior health management executives.
- Plan and conduct outreach and community screening (CST) to the community to promote wellness and health awareness and information with REACH AAC boundaries.
- Conduct/monitor the volunteer orientation, deployment, briefing, and feedback evaluation of befrienders for the department.
- Support annual membership recruitment and renewals for AAC members.
- Plan, develop, and manage engagement programs held at designated neighborhood touchpoints for registered seniors or their caregivers within AAC boundaries. This is mainly for the benefit of buddies/befriendees of RS who are not keen/unable to visit the AAC.

This includes but is not limited to:

- Exercises, educational talks, workshops, group work sessions, family bonding, festive events, online engagement using platforms like Zoom or Facebook Live, etc.
- Maintain proper records of all financial reports and documentation as required by audits, and participate in budgeting for SC’s annual expenditure.

Job Requirements
- Minimum Diploma certification in any discipline with 2 years of relevant experience
- Project Management and Enterprise Management knowledge preferred.
- Resourceful and able to do public speaking when handling events and projects.
- Conversant in English and any 2nd language with ability to converse in local dialect would be an advantage for elderly clients
- A good team-player with a passion to work with Seniors
- Good communication and interpersonal skills

**Job Types**: Full-time, Permanent

**Salary**: $2,580.00 - $3,500.00 per month
