As a Health Champion, you will play a pivotal role in supporting our Seniors of Interest in our communities. This role supports identifying, planning and implementing integrated community care ecosystems to support ageing-in-place; and the execution of the Community of Care (COC 2.0) - Go Broader project. The position will be at AAC-Bukit Merah View or Jurong Central Plaza.
**Planning and Operations**
- Develop and implement outreach plans to engage Seniors of Interest
- Support the Active Ageing Centres (AACs) in reaching out to Seniors of Interest
- Plan, implement and evaluate active ageing programmes targeted for Seniors of Interest
- Support the needs, and empower Seniors of Interest
- Work with the Active Ageing Centres (AACs) in rolling out evidence-based programmes that support Seniors of Interest
**Partnerships Engagement**
- Support the implementation of the new playbook/workflows, document processes, learnings, challenges and solutions, as well as implement any needed improvements for long-term sustainability
- Stocktake and assess the strengths and limitations of existing partnerships and processes, review and optimise existing workflows
- Assist in the development and coordination of the engagement efforts with partners
- Develop an asset map of community partners and resources to identify gaps in existing services and strengths for integrated care partnership engagement
- Work with established partners and government agencies; and attend networking sessions and meetings as required
**Administrative Work**
- Support the Active Ageing Centres (AACs) in data analysis to identify the needs of the community
- Collect and report relevant data for analysis and evaluation, as well as document learnings, challenges and best practices from the pilots
- Ensure accurate record-keeping and report submissions in a prompt and timely fashion
- Support necessary evaluation and all critical reporting processes for the management and funders of the programme
Any other duties as assigned by the reporting officer
May be required to perform additional duties and work over the weekends or after office hours.
QUALIFICATIONS
- Diploma in Social Work, Community Development, Counselling or Psychology preferred
- At least 2 years experience in programme planning, care coordination, community work, or working within mental health settings
- Experience in project management, business development or relationship/stakeholder engagement will be an added advantage
