Case Management Officer

Queenstown, Singapore National University Hospital Full time

Responsible for the administration of the renal retardation programme which provides disease specific case management for patients with chronic kidney disease (CKD Stage 1 to 5).

**Responsibilities**:

- Assess plan, implement and evaluate care of patients with renal disease
- Conduct teaching and counseling for patients and their families / carers
- Conduct / assist to conduct specialty nurse-led clinic for patients and their families where needed
- Co-ordinate patients’ access and treatment sequence where required
- Assist with overall coordination of renal replacement therapy
- Assist with placement of patients in dialysis facilities as required
- Deliver patient care (inclusive of education and counseling) according to the established standards of care and practice using patient care plan / clinical pathways
- Contribute to multidisciplinary patient management
- Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services
- Demonstrate proper use and care of equipment
- Demonstrate proper and economical usage of consumables and supplies
- Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Mangers, Doctors and Paramedical Staff
- Assist to review, develop and evaluate clinical pathways and / or other plans of care in consultation with Specialty Nurses, Nurse Managers, Doctors and Paramedical Staff
- Precept newly joined renal coordinators in delivery of specialty care where needed
- Participate in quality improvement activities
- Take on additional responsibilities or tasks as assigned by the Head of Division
- Maintain a clean and safe working environment

**Job Requirements**
- Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science.
- Registered Nurse with at least 3 years of clinical experience
- Able to demonstrate clinical skills, knowledge and critical thinking in delivery of patients’ care.
- Possess good interpersonal and communication skill
- Computer literacy skills - MS Office (Word / Excel / PowerPoint)
