As a Care Manager (Primary Care/ Community Settings), you will be responsible for the following duties:
- Identify residents in need of care management and support the development and execution of tailored care plans.
- Track residents' daily routines and monitor their overall progress to achieve optimal clinical and non‑clinical outcomes.
- Engage with residents and their families to assess satisfaction with care delivery, communicating feedback accordingly.
- Collaborate closely with clinical teams and relevant stakeholders to ensure high-quality, coordinated care.
- Maintain confidential documentation of patient records; participate in quality improvement initiatives and outreach activities, and fulfill any additional duties assigned.
Requirements:
- Diploma/ Degree in Nursing
- Post Basic Advance Diploma in Nursing (Gerontology) or Chronic Disease Management with a valid Singapore Nursing Board (SNB) registration
