Job description:

This role will lead the Community Care Implementation (CCI) Operations team and oversee all the roles delivered and the drive the projects undertaken by the team This is done through:

  1. Oversee the implementation of Community Mental Health (CMH) programmes under CMH Masterplan:

· Lead the CCI Ops team and oversee and drive the CMH programmes / projects undertaken by the team under the CMH Masterplan

· Lead in the implementation and review of pilots and/or projects with our Service Providers (SPs).

· Work with CCI Ops Managers to set up and maintain structured workflow and processes for claims, report submission, and workplan updates.

  1. Facilitate close collaboration within CCI (Ops and Model of Care) and within same division and/or other divisions in AIC.

· Work with CCI Ops Managers on grounding the programme managers in service model deployment, programme implementation, relationship management and account management.

· Provide feedback and review to CCI MOC and programme teams on the efficacy of service model and interventions (eg training)

· Work with MOC to ensure programme managers are conversant in programme models and feedback from SPs are shared with MOC and actioned upon.

· Proactively look at process improvement to enhance efficiency and productivity for CMH providers and programme managers

  1. Keep abreast of the individual SP's care model and their developmental needs

· Work with programme managers to ensure service model is adhered to.

· Work with other team from internal division to ensure SPs are kept informed and acquires the relevant competencies and attends the relevant case discussions and clinical practice circles.

· Work with CMH providers to identify their learning needs Monitor programme effectiveness through evaluation efforts by coordinating, collating and tracking the outcome measures and performance indicators of CMH programmes

  1. Support SPs in providing centre-based access to mental health and dementia needs.

· Establish a workflow to facilitate service provider's site applications.

· Facilitate and review site applications taking the view of CMH ecosystem by evaluating service provider's site application and programme managers' recommendations.

  1. Plan and implement 'people development' for CCI Ops team

· Design and development programme manager development plan to upskill their programme management

Job Responsibilities

· Bachelor's Degree, Post Graduate Diploma, Professional Degree preferably in Social Work/Psychology/Counselling with 12 years with at least 4 years in a managerial capacity, with experience in mental health, healthcare or community care.

· Experience of working with diverse groups of people in an organization

· Able to multi-task and possess strong analytical, problem solving and project management skills

· A self-starter with strong learning ability, who is resilient, adaptable and able to manage change and ambiguity

· Experience and knowledge in facilitation, with strong negotiation and communication skills
