
Case Management Executive

**As a Case Management Executive, you will be responsible for the following duties**:
- Assess the requirements of seniors who live in communities using tools like the Community Screener, EBAS (for depression), and AMT.
- Examine the care plan with the appropriate parties.
- Encourage the prompt and economical referral and provision of services to seniors
- Organize care in collaboration with company services and outside providers.
- Engage in case management conversations both inside organisation and with outside service providers.
- Create a resource map with services and other points of contact for seniors residing in the center boundary.
- Active aging programs delivered via satellite and on-site Groupwork Outreach events
- Keep records in order for audits and prepare data for submission to MOH and AIC
- Assisting volunteer management executives
- Verify adherence to the policies on finance and human resources.
- Other Ad-hoc assigned
**Requirements**:
- Diploma or Degree holder preferably in community and social services (senior services) or Psychology or Counselling or healthcare services.
- Experience in the social service sector will be an advantage.
- Passionate to work and interact with seniors in the community.
