Executive Case Manager

As a Senior Executive, you will play a pivotal role in providing case management support and counseling services to residents and their families. Your primary responsibility will be to plan, review, and conduct rehabilitative programs for residents, ensuring they receive the necessary care and guidance to overcome challenges.
Furthermore, you will be responsible for preparing, maintaining case files, preparing Individualised Service Plans (ISPs), progress, and closure reports. Additionally, you will refer residents and their families to appropriate agencies for further intervention, analyzing trends and emerging needs of counseling and case management services.
Required Skills and Qualifications:- At least a Degree in Social Work, Psychology, or Sociology from recognized universities.
- Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage.
- Minimum 5 years of relevant social work experience in the field, preferably working with offenders, and with at least 2 years of supervisory experience.
A 5-day workweek, Monday to Friday, from 9am to 6pm, provides a stable and predictable work environment. You will be based at the Halfway House located at 50 Lorong 34 Geylang, Singapore, catering specifically to male residents.
