Home Care Coordinator Job Description
The Home Care Coordinator plays a pivotal role in ensuring the smooth functioning of home care services. This position requires strong organizational skills, effective communication, and a commitment to delivering high-quality patient care.
Key Responsibilities:
- Client Admission and Discharge Coordination: Conduct intake assessments, manage admission and discharge processes, and ensure timely and accurate documentation.
- Service Monitoring and Staff Rostering: Oversee daily personal care services, coordinate staff scheduling, and maintain comprehensive records.
- Operations Support and Administrative Management: Provide day-to-day operational support, maintain records, and update documentation as needed.
- Staff Development Support: Assist in implementing training programs and competency frameworks to enhance service quality and staff performance.
- Client and Caregiver Engagement: Obtain regular feedback, assess satisfaction, and identify areas for improvement.
Required Skills and Qualifications:
- Diploma or Degree in a relevant discipline.
- Excellent communication and interpersonal skills.
- Team player with strong organizational and meticulous work habits.
- Proficient in Microsoft Office.
- Good understanding of working with elderly populations.
