Home Care Coordinator Job Description

The Home Care Coordinator plays a pivotal role in ensuring the smooth functioning of home care services. This position requires strong organizational skills, effective communication, and a commitment to delivering high-quality patient care.

Key Responsibilities:

  • Client Admission and Discharge Coordination: Conduct intake assessments, manage admission and discharge processes, and ensure timely and accurate documentation.
  • Service Monitoring and Staff Rostering: Oversee daily personal care services, coordinate staff scheduling, and maintain comprehensive records.
  • Operations Support and Administrative Management: Provide day-to-day operational support, maintain records, and update documentation as needed.
  • Staff Development Support: Assist in implementing training programs and competency frameworks to enhance service quality and staff performance.
  • Client and Caregiver Engagement: Obtain regular feedback, assess satisfaction, and identify areas for improvement.

Required Skills and Qualifications:

  • Diploma or Degree in a relevant discipline.
  • Excellent communication and interpersonal skills.
  • Team player with strong organizational and meticulous work habits.
  • Proficient in Microsoft Office.
  • Good understanding of working with elderly populations.
