Home Care Service Manager

The role of a Home Care Coordinator is crucial in ensuring the smooth functioning of home care services. This position plays a vital part in supporting the delivery and operations of these services.
This includes working closely with the Staff Nurse and other team members to ensure administrative processes, client care coordination, and service quality are maintained at high standards.
In addition, this role is essential in maintaining operational efficiency within the home care setting.
Duties and Responsibilities:- Conduct intake assessments, including basic financial counselling and service agreement.
- Manage the admission and discharge process for home care referrals, ensuring timely and accurate documentation.
- Oversee the daily provision of personal care services to clients.
- Coordinate staff scheduling, including rostering and leave management, to ensure adequate coverage and continuity of care.
- Provide day-to-day operational support and maintain comprehensive and up-to-date records for all clients.
- Assist in the implementation of training programs and competency frameworks for care staff to enhance service quality and staff performance.
- Obtain regular feedback from clients and caregivers to assess satisfaction and identify areas for improvement.
- Address concerns and escalate issues appropriately to ensure client well-being.
- Perform any other duties as assigned by the supervisor to support the overall functioning of the home care services.
- Diploma / Degree in any relevant discipline.
- Good communication and interpersonal skills.
- Team player who is organized and meticulous at work.
- Proficient in Microsoft Office.
- Good understanding of working with elderly.
