Community Care Specialist

Overview
We are seeking a dedicated and compassionate Community Care Specialist to join our team. As a key member of our community healthcare services, you will play a vital role in enhancing the lives of individuals and their caregivers.
The ideal candidate will have a strong background in nursing, mental health, or a related field, with experience working in community settings. You will be responsible for providing high-quality care coordination, psycho-education, and support to clients and their families.
Key Responsibilities
- Conduct home visits to assess patients' medical, psychological, social, environmental, and vocational needs
- Collaborate with Allied Health Professionals, doctors, and nurses to ensure holistic care
- Offer psychological expertise in multi-disciplinary meetings
- Provide training for team members and caregivers
- Facilitate Mental Health support groups for patients and caregivers
Requirements
- Registered Nurse with the Singapore Nursing Board
- RMN/Advanced Diploma in Mental Health or relevant qualifications (e.g., social work, counseling, psychology)
- 3 to 5 years of psychiatric experience (Community psychiatric clinic/IMH preferred)
- Skilled in psychotherapy for mental health patients
- Strong organizational, interpersonal, communication, and writing skills
- Proficient in MS Office and familiar with healthcare environments
- Ability to multi-task, work independently, and collaborate effectively
