Job Title:
Community Care Specialist

About the Role:
We are seeking a highly skilled and experienced Community Care Specialist to join our team. As a key member of our community care initiatives, you will play a vital role in improving care integration and delivery in the community.

Key Responsibilities:
  • Establish and maintain strong relationships with key account community partners to enhance care coordination and delivery.
  • Manage feedback and issues from community care partners in a professional and timely manner.
  • Stay up-to-date with developments within ILTC and community care organizations to ensure our initiatives remain effective and relevant.
  • Identify opportunities for collaboration with key partners to improve care integration and augment community care services.
  • Lead the management of collaborations/initiatives/projects to enhance care integration between NUHS and ILTC providers and/or augment community care services.
  • Provide administrative support as a workgroup/meeting secretariat.
  • Support the implementation of NUHS Population Health strategies and regional health plan.

Requirements:
To be successful in this role, you will need:
  • A good degree in Life Sciences, Business Administration or a related field.
  • 3-5 years of relevant working experience, preferably in community care or partnerships.
  • Proven project management skills and experience working with multiple stakeholders.
  • The ability to engage confidently with internal and external stakeholders at all levels.
  • Excellent analytical, organization, and communication skills.
