Community Care Specialist

Job Description:
As a Community Care Specialist, you will be working closely with Medical Social Workers to conduct home visits and obtain collateral information about patients' physical and familial environments. You will also assist in interviews and assessments of patients and their family members for appropriate service referrals.
You will be responsible for registering all referrals received by the department, conducting intake interviews for MSWs, and performing means-testing for patients seeking subsidies for community hospital services as stipulated by the Ministry of Health.
In this role, you will have the opportunity to work independently while being part of a team, utilizing your interpersonal skills to provide excellent customer service.

Required Skills and Qualifications:
  • Diploma Holders (preferably in Community Services or Human Services) with 4 years of relevant experience
  • GCE 'O' or 'A' level preferably with 10 years of relevant experience
