  • Conduct home visits as necessary, ensuring the well-being of vulnerable seniors through the Alert Alarm System (AAS).
  • Administer Community Screener Tools to assess the needs of cluster seniors, promoting early intervention and support.
  • Provide valuable information and referral resources to seniors, empowering them to make informed decisions about their care.
  • Offer administrative assistance as required, maintaining a high level of organisational efficiency within the team.
