Support Worker

We are seeking a Support Worker to join our team. The successful candidate will be responsible for providing direct support to clients, facilitating their personal growth and well-being, and ensuring they receive the necessary assistance to live independently.

Job Description:

The Support Worker will be responsible for:

  • Providing one-to-one support to clients, enabling them to maintain their independence and engage in activities of daily living;
  • Facilitating client-centred plans, working collaboratively with multidisciplinary teams to ensure client needs are met;
  • Conducting risk assessments, identifying potential risks, and developing strategies to mitigate these risks;
  • Coordinating services with external agencies, ensuring seamless care delivery;
  • Maintaining accurate records, documenting client progress, and contributing to quality improvement initiatives;
Required Skills and Qualifications:

To be successful in this role, you will need:

  • A degree in Social Work or related field;
  • At least 1-2 years' experience in social work, preferably in a residential setting;
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues;
  • A patient and empathetic approach, with a commitment to delivering high-quality care;
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands;
