Senior Dementia Care Specialist

We are seeking a skilled Senior Care Professional to join our team. The successful candidate will be responsible for conducting case assessments, formulating individualized care plans, and providing support to individuals with dementia and their carers.
The role involves working collaboratively with multidisciplinary professionals and external service providers to ensure holistic care management. Key responsibilities include:
- Conducting thorough case assessments and creating personalized care plans
- Providing psychoeducation, supportive counseling, and referral services as needed
- Liaising with healthcare professionals and community organizations to coordinate care
- Maintaining accurate records and documentation
- Participating in team meetings and contributing to program evaluation
To be successful in this role, you will require:
- A degree in an allied health profession, social work, or psychology
- Excellent communication and interpersonal skills
- Ability to facilitate group sessions (desirable)
- Flexibility to work occasional Saturdays and weekday evenings
- Passion for dementia care and working with individuals
