Job Role
The Senior Social Worker will be responsible for delivering case management services to individuals with dementia and their caregivers.

Responsibilities:
  • Conduct intake assessments and develop care plans.
  • Coordinate services with external agencies.
  • Provide casework support, including information and referrals, and liaise with external agencies.
  • Liaise with colleagues within social services and professionals in hospitals/agencies/organization to commission appropriate services.
  • Keep accurate written records and produce reports, as required.
  • Organise and facilitate activities for carers and persons with dementia.

Essential Requirements

  • A degree in Social Work and registration as an Accredited Social Worker.
  • Case management experience, particularly working with individuals with dementia and caregivers.
  • Ability to work effectively as part of a team.
  • Discretion and attention to detail when dealing with sensitive and confidential information.
  • Experience working in dementia care, geriatrics or mental health settings is highly desirable.
