Job Title: Skilled Home Care ProfessionalAbout the Role

We are seeking a skilled home care professional to join our team in providing palliative care services to patients in their homes. Our goal is to deliver high-quality, patient-centered care that meets the unique needs of each client.

  • Key Responsibilities:
  • Provide hands-on bedside care, administer medication, and perform wound care as needed.
  • Work collaboratively with our multidisciplinary team to develop and implement individualized care plans.
  • Assist with respite assignments and provide support to family members and caregivers.
  • Conduct hospital or home visits to assess new clients for home care services.

Requirements:

  • A minimum Diploma or Bachelor's Degree in Nursing from an accredited institution.
  • A valid registration with the Singapore Nursing Board and a clean disciplinary record.
  • Proficiency in Microsoft Office and comfort with IT systems used in healthcare settings.
