Delivering Compassionate Nursing Services

As a Staff Nurse in Palliative Home Care, you will have the opportunity to make a positive impact on patients' lives. Our team is seeking a dedicated and compassionate individual to provide high-quality care to patients in their homes.

Key Responsibilities:
  • Provide hands-on bedside care to patients, assessing their needs, developing care plans, and implementing interventions.
  • Take up respite assignments when required, providing temporary relief to caregivers and ensuring continuity of care.
  • Assist the home care team on other home nursing cases, collaborating with colleagues to ensure seamless care delivery.
  • Advise care staff as needed if they face medical issues or medical emergencies with clients, providing expert guidance and support.
  • Conduct hospital or home visits to assess new clients for home care services or when required, ensuring accurate diagnoses and effective treatment plans.
  • Work together as a team to handle complaints and crisis management, maintaining a positive and supportive work environment.
  • Collaborate with external community partners and colleagues in managing client care needs, promoting holistic care and addressing social determinants of health.
  • Ensure strict compliance to documentation as required by regulatory standards, maintaining accurate records.
Requirements:
  • Minimum Diploma or Bachelor's Degree in Nursing from an accredited institution.
  • Must be a Registered Nurse with a clean disciplinary record.
  • Proficiency in Microsoft Office and comfortable with IT would be ideal.
  • Occasional shift work (to support palliative patients) will be required when necessary.
