We are seeking a skilled and compassionate Home Nursing Services Provider to join our team. The successful candidate will be responsible for providing high-quality home nursing services to clients and supporting their caregivers.
About the RoleThe Home Nursing Services Provider will work collaboratively with internal and external key stakeholders to facilitate effective care of clients, enabling them to age in place of their choice. Key responsibilities include:
- Conducting home visits to assess and develop personalized nursing intervention and care plans for clients.
- Performing nursing procedures in the home environment while prioritizing client safety.
- Providing patient education to promote improved health outcomes.
- Liaising with healthcare partners to ensure efficient and effective care planning.
- Training caregivers on various aspects of client care.
- Carrying out care and discharge processes upon admission.
