Job Title: Dementia Care Specialist

Job Description
  • We are seeking a skilled Dementia Care Specialist to join our team. As a key member of our care coordination team, you will work closely with individuals living with dementia and their caregivers to provide support and resources.
  • Key responsibilities include conducting case assessments, formulating individualized care plans, and providing interventions such as psychoeducation and supportive counselling.
  • You will also be responsible for attending case conferences, care planning and review meetings, and developing carer peers through peer support networks.
  • In addition, you will participate in outreach events to promote dementia awareness and deliver public education talks.
  • Able to keep accurate written records and produce reports as required.
Required Skills and Qualifications
  • Degree in Social Work and registration as an Accredited Social Worker.
  • Keenness in doing case management work with persons with dementia and carers.
  • Team player with attention to detail and ability to handle sensitive and confidential information.
