Job Description">

As a dedicated social worker, you will play a vital role in providing comprehensive support to individuals with dementia and their caregivers.

You will conduct thorough assessments, develop personalized care plans, and implement interventions such as psychoeducation, counseling, and service referrals. Additionally, you will liaise with multidisciplinary professionals, provide case recordings, and participate in team meetings and conferences.

Responsibilities include planning and facilitating caregiver support group sessions, assisting in data preparation, and contributing to program evaluation and reporting.

  • Conduct case assessments and formulate Individualized Care Plans for Persons with Dementia and their carers.
  • Provide intervention such as psychoeducation, supportive counselling and service referral.
  • Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
  • Provide case recordings and relevant documentations.
  • Participate in team meetings and case conferences as required.
  • Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
  • Plan and facilitate Caregiver Support Group sessions.
  • Support in outreach events and talks.
Required Skills and Qualifications

To be successful in this role, you will possess:

  • A degree in an allied health profession, social work, or psychology.
  • Excellent oral, written, and interpersonal communication skills.
  • The ability to facilitate group sessions.
  • Flexibility to work occasional Saturdays and weekday evenings.
  • Passion for dementia care and working with people.
  • Prior experience in dementia care is advantageous.
