Job Description\

We are seeking a skilled Family Support Specialist to join our team. This role plays a key part in assisting individuals with intellectual disabilities and their families during life transitions, such as moving from school to the community or between residential and community care.

The ideal candidate will have excellent communication skills, be able to work closely with a multidisciplinary team, and possess experience working with individuals with intellectual disabilities or special needs.

Key Responsibilities:\
  • \
  • Coordinate and conduct yearly family meetings to review person-centered goals and facilitate inclusive discussions where everyone's voice is heard.\
  • Support families and caregivers through transitions, home visits, and community assessments, empowering them with skills for daily living and community integration.\
  • Collaborate with transition support officers and training officers to align goals, strategies, and skill transference from structured settings to real-life environments.\
  • Provide case management and guide families through transitions such as school-to-work or residential-to-community care.\

Required Skills and Qualifications:\
  • \
  • Minimum diploma in social work, psychology, special education, or other relevant fields with at least 2 years of working experience.\
  • Excellent written and verbal communication skills.\
  • Experience working with individuals with intellectual disabilities or special needs.\
  • Ability to speak an additional local language apart from English.\
  • Strong interpersonal, organisational, time management, and problem-solving skills; self-motivated and adaptable.\
