About the Role

We are seeking a skilled and compassionate Social Worker/Case Manager to join our team.

This is an exciting opportunity for a dedicated professional to make a tangible difference in people's lives by providing case management, conducting psycho-social assessments, and identifying common needs and goals for intervention.

The ideal candidate will have excellent interpersonal skills, be able to work in a time-pressured environment, and possess strong analytical and problem-solving abilities.

Main Responsibilities:

  • Manage cases of different risk levels.
  • Conduct risk assessments related to family violence, child abuse, and suicide.
  • Develop safety plans with family and network members.
  • Provide basic counseling to clients as required.
  • Plan, organize, and implement community outreach activities, programs, and group work.
  • Evaluate and prepare reports for administrative investigation and intervention programs.
  • Provide information and referral services to link clients to appropriate social services.

Requirements:

  • Possess a degree in social work or a graduate diploma in social work from an accredited institution.
  • Be registered as a social worker with the Singapore Association of Social Workers (an advantage).
  • Have 1-2 years of relevant experience in social service agencies.
