The Case Manager for the Youth Service plays a pivotal role in driving positive change for individuals, families, and communities facing transitional challenges. This position involves providing direct services to clients and facilitating rehabilitation and reintegration efforts.

The primary scope of this role focuses on the Reformative Training Centre (Singapore Prison Service), with opportunities to engage in community outreach programs, the Singapore Boys’ Home, and other initiatives assigned by the Head of Department.
The Case Manager will be responsible for developing and implementing intervention plans based on professional knowledge, conducting casework, and leading various community development activities.
**Key Responsibilities**:
- Build and maintain professional relationships with clients and their families/caregivers.
- Conduct intake assessments to identify client needs and develop tailored intervention plans.
- Manage case plans, including regular reviews, follow-ups, and accurate documentation.
- Facilitate group programs, including psycho-educational and therapeutic sessions.
- Collaborate with external agencies for effective case management.
- Supervise and mentor junior staff and interns.
- Participate in professional supervision and undertake additional duties as assigned.
**Requirements**:
- A minimum of a graduate diploma or degree in Social Work, Counselling, Psychology, Sociology, or a related field. A diploma in the same field with strong relevant experience may be considered.
- At least two years of case management experience, with a focus on at-risk youth, is highly preferred.
- Ability to work within time-based programs to achieve desired outcomes.
- Self-motivated, able to work independently, and an effective team player.
- Excellent interpersonal and communication skills for working with youth-at-risk, young offenders, and their families.
- Skilled in case assessment, engagement, intervention planning, and prompt reporting.
- Experience designing, planning, implementing, and managing group work and programs, particularly in prison, youth centre, or residential facility settings.
- Knowledge of community trends and best practices to improve program impact.
- Preferably trained in FAST (Family and Adult Support Tool) and/or YLS/CMI (Youth Level of Service/Case Management Inventory).
- Proficiency in English; ability to converse in Mandarin and/or Malay is a plus.
